fix(tui): bubble nested subagent asks up to the root - #64
No issue. Requested directly: asks from nested subsessions should bubble up to the root, and every view that lists subsessions should show them nested.

What does this PR do?
Subagents can now launch their own subagents, so session trees go root → child → grandchild. The TUI shows every ask (permission request or question) on the root session's view, but it only knew a session's direct children after a start or reconnect. An existing grandchild's ask was invisible at the root until you opened its parent; you heard the sound and had nothing to answer.
The session and its direct children load exactly as before, failures included.
Below that, the walk is best effort: a branch whose listing fails is left to live events instead of failing the sync and closing the view.
At most 8 listings run at a time, so a wide tree doesn't queue the client's other requests.
A seen set lists each session once and ends the walk on a parent cycle. The cost is one list request per session in the tree, plus one per extra 200-child page; repeat calls are free until a reconnect.
The server filters sessions only by direct parent, so a subtree query would mean a protocol change, and an older server could ignore the filter and return every session.
Asks at the root.
Pending permissions and forms load for every session in the family, so the root view shows all of them after a fresh start.
A subagent view opened before its ancestors were loaded loads up to the root, then the root's tree.
Tabs. A background tab loads pending asks for every family member, so its attention marker is right after a restart.
Subagent picker. The active/inactive filter now applies while the tree is built. A filtered-out parent of a match stays as a context row, connectors are drawn for visible rows only, and a parent cycle no longer loops forever. The root is still not listed, as before.
Notifications. A session this TUI hasn't loaded is fetched before it is announced, so a nested subagent stays sound-only (subagent_done) and gets its title. Before, it was treated as a root.
Views that list only root sessions or count family members needed no change and now see complete families. The opencode mini "Select subagent" menu is still flat and activity-sorted; nesting it would change its search and cycling, so it's left for a follow-up.
How did you verify your code works?
Client (packages/client), each failing before the fix:
a deep tree is published level by level with one list request per session;
a parent cycle ends after 3 requests;
a failing nested listing keeps the rest of the tree with at most 8 listings in flight;
the 250-child paging test now also expects one nested listing per child.
Full client suite: 246 pass.
TUI (packages/tui), each failing before the fix:
session-nested-asks.test.tsx, which runs the whole app: root → child → grandchild with a pending permission and form on the grandchild. A fresh root view shows both, and Enter answers both against the grandchild. A grandchild view opened cold lists the whole family in the picker.
session-tabs.test.tsx: a background tab's marker lights up for asks from a grandchild and a great-grandchild after a restart.
util/session.test.ts and composer-keymap.test.tsx: the filtered picker keeps parents as context and draws the right connectors.
notifications.test.ts: an unloaded session is fetched before it is announced.
data.test.tsx: the test that pinned one-level loading now expects the whole tree.
Full TUI suite: 1495 pass, 3 fail. All three are in plugin-source.test.ts and also fail alone on the unchanged base.
The new tests passed 10/10 reruns.
bun typecheck in packages/client and packages/tui, and bun run check at the root: pass.

Summary by CodeRabbit
New Features
Session syncing discovers descendants at every depth, with sessions appearing as they load.
Session views and background tabs include permission and form requests from across the session family, including nested sessions.
Subagent filtering preserves parent context, and notifications load session details before appearing.
Opening a nested session can discover its full family, with retries after temporary loading failures.
Bug Fixes
Session-family traversal safely handles cycles, and a failure to load one nested session no longer prevents other descendants from appearing.
Outdated completion and request notifications are suppressed when newer activity takes precedence.
Subagents can launch their own, so syncing a session with children now walks its whole subtree breadth first: one paged list per session, each level published as it arrives, and a seen set so a parent cycle still ends the walk. The family index, root costs and everything built on them now include grandchildren on a fresh client instead of only direct children.
The session view syncs permissions and forms for every member of the viewed family, so the root view prompts for asks from subagents at any depth and every view of the family marks its tab. A subagent view opened before its ancestors walks up to the root and loads the root's whole tree. Background tabs also load their nested subagents' pending asks, so a fresh start marks them.
The active filter ran after the tree prefixes were drawn, so a running subagent under an idle one showed a connector to a missing row and siblings kept the wrong corner. Filter while building the tree instead: a filtered-out ancestor of a match stays as context, connectors are drawn for the visible tree, and a parent cycle no longer loops forever.
An event from a session this client never loaded, such as a nested subagent of a root that is not open, was announced as a root session: a desktop notification, the done sound and no title. Load the session first so subagents stay sound-only and every notification has its title.

📝 Walkthrough
Session sync now discovers descendants recursively. The TUI uses session families to sync nested permission and form data, filter subagents, and defer notifications until referenced sessions load.
Session sync discovers descendants at every depth, uses a shared limit of eight concurrent listings, publishes eligible sessions, and avoids repeated listings. Tests cover listing failures, concurrency, stale responses, cycles, and reconnect refreshes.
Family tree filtering packages/tui/src/util/session.ts, packages/tui/src/routes/session/composer/subagents-tab.tsx, packages/tui/test/util/session.test.ts, packages/tui/test/cli/tui/composer-keymap.test.tsx
sessionFamily accepts an inclusion predicate, retains matching sessions and their ancestors, and stops traversal at cycles. The subagents tab applies activity filtering while building the family tree.
Family session synchronization packages/tui/src/routes/session/family.ts, packages/tui/src/routes/session/index.tsx, packages/tui/src/context/session-tabs.tsx, packages/tui/test/cli/tui/session-family.test.ts, packages/tui/test/session-nested-asks.test.tsx, packages/tui/test/context/session-tabs.test.tsx
The session route discovers families with retries and syncs permissions and forms for family members. Delayed tab prefetch syncs those requests for each family member. Tests cover discovery retries, nested asks, family navigation, and tab attention state.
Notifications for unloaded sessions packages/tui/src/feature-plugins/system/notifications.ts, packages/tui/test/cli/cmd/tui/notifications.test.ts
Notifications wait for session sync when an event references an unloaded session. Completion and failure callbacks check that the execution is current; form and permission callbacks check that the request remains pending.

Merge Risk:🟡 Moderate · up to d1e9b
A temporary failure while listing a nested session can leave its descendants and pending asks undiscovered on later sync calls. Resolve that retry gap before merging unless the limitation is explicitly accepted.
Security architecture risk:🔵 Low · up to d1e9b
The change automatically brings deeper subagent requests into the root view. Request identities and server access controls remain in place. Remaining uncertainty concerns reconnect interleavings, compatibility, and imported session trees that span locations.
Retained concerns
No architecture-level concerns identified.
Security review details
Security Blast Radius
inferred — Automatic discovery broadens the effective root approval scope to previously unloaded descendants on the connected server. The existing autoaccept mode consequently reaches newly discovered pending requests, including cross-location descendants if imported parent links form such a tree. This does not itself establish a tenant escape: the inspected authentication boundary is server-wide, and replies retain the originating session and request identities.
Trust Boundaries and Controls
observed — The API applies authorization middleware, and the normal server startup requires a password. Authorization accepts instance credentials or a verified session cookie, with an origin check for cookie use. Session-location middleware selects the target session's execution context rather than substituting the root session's location.
Resilience and Maintainability Implications
observed — Deferred permission and form notifications recheck whether their request remains pending after session loading. Deferred completion notifications use per-execution symbols to suppress notifications superseded by another execution.
Hardening Proposals
proposed — If a session family is intended to define an approval-policy boundary, validate imported parent/location consistency or make cross-location origin explicit before applying root autoaccept. This is hardening for an existing permissive import model, not a verified authorization bypass.

The eight-listing limit applied per walk, and session tabs sync every open tab's family at once, so several walks could still queue the client's other requests behind discovery. One limiter per data instance now caps descendant listings across every walk, and each listing publishes its children as soon as it resolves instead of waiting for the rest of its level.
A family listing could publish a descendant's snapshot after a live rename had already updated it, and the settled session read then made later syncs no-ops, so the stale title stuck. A listed snapshot now only fills in sessions this client has not read; a session whose own read is settled or in flight stays current through that read and live events.
A notification waiting for an unknown session to load could still fire after another client answered the permission, replied to or cancelled the form, or after a new execution started. The deferred callbacks now recheck that the ask is still pending and that their completion is still the session's latest.
A transient failure while a subagent view walked up to its root was swallowed, and nothing reran the walk until a reconnect, so the view stayed without its family. The walk now lives in createFamilyDiscovery and retries a failed read after 250 ms, doubling up to five times; a reconnect or a new view starts over.

Keep failed descendant branches eligible for retry.
If a descendant listing fails, syncDescendants converts that failure to an empty child list. This family sync then resolves and sync.run caches it as complete. A later session.sync(root, { children: true }) cannot retry the branch, so an existing deeper session and its pending asks can remain absent until another cache invalidation. Track incomplete discovery separately so the current call can remain best effort while a later call retries failed branches.

🔵 Needs a closer look
Deferred notifications can survive plugin cleanup, and reconnect discovery can overwrite newer live usage data.

Prevent stale listings from overwriting live usage updates
packages/client/src/solid/data.ts:601
After reconnect invalidates a cached grandchild, a live session.usage.updated event can update its cost and tokens while its parent's listing is pending. Unlike the rename handler, the usage handler (lines 776–782) does not sync the session, so unread() still accepts the older listing. Publication then overwrites the live values and marks them current, leaving family cost totals stale. Both operations occur in the same generation, so the generation guard cannot prevent this. Preserve live field updates while refreshing metadata missed offline, and add a regression for this ordering.
Clear pending notification state during plugin cleanup
If the notifications plugin is disabled while this session read is pending, its callback can still play a sound or send a desktop notification after cleanup. The disposer removes listeners but leaves forms, permissions, and terminal populated, so deferred ask and completion callbacks still pass their checks. Clear those collections during cleanup to invalidate pending announcements.

@coderabbitai The retry gap from your merge-risk note is fixed in a6fa715. The walk now reports whether every nested listing succeeded. If any failed, the session's subtree sync is not cached, so the next sync(id, { children: true }) lists the tree again instead of returning the stale result.
What triggers the retry: the TUI and the app re-sync a family when a session view opens and on reconnect, and TUI tabs re-sync when they prefetch. Live events only update fields of a session's info and never replace it, so a branch that keeps failing cannot cause a re-sync loop.
Live asks: a descendant under a failed branch that raises a new ask is loaded by its event and indexed under its known parent, so the ask still reaches the root.
Test: "lists a branch again on the next sync after its listing failed" in packages/client/test/solid-data.test.ts. A grandchild listing fails once, and the second sync finds the grandchild. It failed before the change.
Checks: client suite 249 pass, full TUI suite 1507 pass, typechecks and bun run check pass.

@dezren39 Thanks for the clarification. I reviewed a6fa7153ce and the regression test.
syncDescendants now propagates nested-listing failures.
An incomplete walk invalidates session.family:${sessionID}, so the next family sync can retry discovery.
The test verifies that the first sync misses the grandchild and the second sync discovers it.
This addresses the retry gap from my merge-risk note. I did not rerun the checks; the pass counts are from your report.

Learnt from: dezren39
URL: https://github.com/developing-today/opencode/pull/64
Timestamp: 2026-10-18T17:36:51.208Z
Learning: In developing-today/opencode, session-family discovery retries occur when TUI or app session views open, on reconnect, and during TUI tab prefetch. Live session-info events update fields without replacing the info object, so repeated nested-listing failures do not themselves trigger a reactive family-sync loop. In packages/client/src/solid/data.ts, incomplete descendant walks invalidate the session.family cache entry so a later sync(id, { children: true }) retries discovery.

No issue. Requested directly: asks from nested subsessions should bubble up to the root, and every view that lists subsessions should show them nested.

What does this PR do?
