chore(deps): upgrade runtime deps, dev toolchain, and Node floor - #730
Conversation
Runtime: hono 4.13.13, ws 8.22.0, zod 4.6.5, undici 8.11.2. Toolchain: vitest 5.0.2 (+coverage/ui), eslint 10.12.0, typescript-eslint 8.71.1, lint-staged 17.6.0, fast-check 4.10.2, @types/node 26, typescript-language-server 6.0.1, eslint 10.x plugin updates. TypeScript is split deliberately: `typescript` stays on 6.0.3 for the JS compiler API (typescript-eslint peer <6.1, tsserver, tests importing `typescript`), while `tsgo` (npm:typescript@^7.0.2) supplies the TS7 native compiler so `.bin/tsc` is tsgo. engines.node moves to >=22.19.0 (undici 8 and vitest 5 both require Node >=22; Node 20 is EOL). CI matrix becomes 22.x/24.x, the smoke job moves from node18-smoke to node22-smoke, .nvmrc -> 22, and all GitHub Actions pins move to current SHAs (checkout v7, setup-node v7, upload/download-artifact v7/v8, codeql-action v4.38.2, hol scanner). Source fixes required by the upgrades: - lib/model-route-policy.ts: annotate the merged catalog entry as RouteModel; the TS7 native checker infers the spread too narrowly. - lib/request/url-rewriting.ts: pass allowH2:false to ProxyAgent so undici 8's H2-by-default ALPN negotiation can't change the upstream wire protocol versus the 6.x line. SECURITY.md pin rationale updated for hono 4.13.13, brace-expansion 5.0.12, and undici 8.11.2; docs/AGENTS version refs updated to the new Node floor. npm audit: 15 -> 0 vulnerabilities. `codex-multi-auth` is a multi-account OAuth manager for the official `openai/codex` CLI. It keeps a named pool of ChatGPT sign-ins on your machine, lets you switch the active account, and — when you run Codex through its optional wrapper — rotates accounts across live requests through a loopback-only proxy. It never installs or shadows a `codex` binary; the official OpenAI install keeps owning that command.

## Runtime rotation, in one paragraph
With rotation on (the default), request-bearing wrapper commands — `exec`, `review`, `resume`, `fork`, `app`, and the bare TUI — route through a local proxy that picks the healthiest account per request, refreshes tokens as needed, and fails over across the pool on rate limits within a bounded retry budget. The proxy listens on loopback only, authenticates its own clients with a per-process token, forwards only Responses API, model-discovery, image, and thread-goal calls, and never writes account emails or tokens to client headers or logs. Plain `codex` is never touched: rotation exists only inside `codex-multi-auth-codex`/`mcodex` sessions, or inside the packaged desktop app after an opt-in, reversible bind. Turn it off with `codex-multi-auth rotation disable` or `CODEX_MULTI_AUTH_RUNTIME_ROTATION_PROXY=0`.

Otherwise → `codex_runtime_rotation_pool_exhausted` (503/429 JSON) carrying `reason`, `retry_after_ms`, and `account_skip_reasons`, and pointing at `codex-multi-auth rotation status`. Bounded always: fetch timeout 60s, stream stall 45s, capacity deadline, 16-iteration pin ceiling — never a silent hang.

## Runtime Rotation Transport Selection
The app-server CLI shim is deliberately **not** installed on the `app-server` branch. Its only purpose is routing a Codex-spawned `codex app-server` (the desktop app via `CODEX_CLI_PATH`) back through the wrapper; installing it stamps `CODEX_MULTI_AUTH_RUNTIME_ROTATION_PROXY=0` onto an environment that a resident server passes to shell tools and MCP servers — every nested `codex-multi-auth-codex` call would read rotation as disabled and bill the wrong account. With no shim there is no `CODEX_MULTI_AUTH_APP_SERVER_ACCOUNT_LABEL`, so the branch requests account-response rewriting explicitly via `proxyAppServerAccountRead` (load-bearing; dropping it fails `test/codex-bin-wrapper.test.ts` account-response tests).

## Runtime Rotation Transport Selection
A helper that cannot start is a hard failure on all canonical-home branches — unlike the shadow path there is no rotation-off shape to degrade into, and quietly serving a resident server unrotated is worse than not serving it. `createRuntimeRotationProxyContextIfEnabled` catches the launch failure, releases the compatibility home already built, and returns a `startupError` that `forwardToRealCodex` converts to exit 1 *before* the official CLI spawns. On the shadow path, proxy failure degrades to plain forwarding unless an explicit upstream demands rotation.

## Runtime Rotation Transport Selection
Helper self-reaping is identity-checked and bounded — a detached helper decides "is my launcher alive" by PID **plus kernel start time** (`CODEX_MULTI_AUTH_APP_ROTATION_OWNER_START_TIME_MS`), and reaps itself on the first of these deadlines:

| Detached window | `CODEX_MULTI_AUTH_APP_ROTATION_DETACHED_IDLE_MS`, default 15m, `0` disables. Applies once the owner is *confirmed* dead, and only to a helper that has **never served a request** (`totalRequests: 0`) with zero open connections — a live `codex app` session idle between turns holds no socket (proxy `keepAliveTimeout` stays at Node's 5s), so socket-only gating would reap a working proxy. ## Runtime Rotation Transport Selection
Helper lifecycle metadata: each helper publishes `runtime-rotation-app-helper.<pid>.json` (un-suffixed legacy path still read) on change plus heartbeat, owns a `runtime-rotation-app-helper-owner.<pid>.json` identity file removed on exit, and each launcher sweeps dead-PID metadata *after* spawning its own helper (the sweep is synchronous and unbounded and must never sit in front of `codex app`/TUI startup). `rotation unbind-app` reclaims the same metadata independently — the only repair path on a machine that stopped launching helpers. Terminal states are `idle-timeout`, `owner-gone`, `max-lifetime`, `stopped`, `error`; only `running` means running. `idleExpiresAt` reports whichever deadline is actually enforced.

Two interactive sessions can run concurrently against the same canonical home — same as running official Codex twice — and **no lock is taken over session state** (neither session copies or syncs it). Scope that guarantee to session state only: `ensureCodexCliFileAuthStore` still read-modify-writes canonical `config.toml` when the store is not `"file"`, safe in practice because it is idempotent and lands via atomic rename — anything non-idempotent added there would need a real lock.

## CONVENTIONS
- Module dependencies must stay acyclic (enforced by `import-x/no-cycle` in lint) and follow the layering
`types/constants → storage → accounts → runtime → manager/CLI`: lower layers never import from higher ones.
Shared types/helpers belong in the lower layer (e.g. `storage/public-types.ts`), with higher layers re-exporting
for surface compatibility instead of lower layers importing back from facades like `lib/storage.ts`.

functions are reserved for genuinely process-global concerns (auth rate-limit trackers, the routing mutex,
UI runtime options, the storage-meta cache, the JSON-store write queues, the runtime policy caches) and must ship
a test reset helper (`reset*ForTests` / `resetVolatileRuntimeState`-style, e.g. `resetRuntimePolicyCacheForTests`,
