It builds on the tool-annotations work (idempotentHint) to add structured failure classification for CallToolResult, and the schema-level prototype is already up in #3312. discussion #2930. Placeholder SEP number (0000) per SEP-1850 -- will rename
the file to match this PR's number once assigned, and update the header
accordingly.
