Each build of VSIX should have a unique version

* [ ] Update version to contain a unique (incrementing) build number
* [ ] Ensure version number of extension in `package.json` is in sync with version number in `changelog.md` for releases
* [ ] Review and update https://github.com/Microsoft/vscode-python/blob/master/CONTRIBUTING.md#versioning
