feat(acp): follow every nested subsession and its asks between prompts - #58

What does this PR do?
opencode acp runs OpenCode behind an ACP client such as Zed. The client sees one session, and OpenCode routes that session's subsessions and their asks (permission requests and questions) to it. Until now ACP followed only subsessions created during the current prompt, and only until they finished. With nested and continued subagents, that drops asks, and the asking session waits forever:
A child continued in a later prompt (the subagent tool with sessionID) emits no session.created, so ACP never saw it or anything it launched. Their permission requests never reached the client, and the prompt hung.
After a prompt, ACP followed only children still running and ignored the session's own asks. An ask from a descendant woken later, or from the session itself when a background result woke it, reached nobody.
Asks already pending when a prompt started, or when a session was loaded, were never shown.
Changes:
Lineage. When an event names a session the prompt doesn't know, ACP walks parentID up to the prompt's session or a tracked descendant. That costs one GET /api/session/:id per unknown session, and the answer is kept for the subscription. Descendants at any depth are followed with the right depth, including ones from earlier prompts.
Between prompts. After a prompt, ACP keeps following the session until its next prompt or session/close, including the session's own asks. The next prompt stops the follower. Requests the client still has open are cancelled with $/cancel_request and asked again.
Pending asks. When a prompt starts, and after session/load or session/resume responds, ACP lists the location's pending permission requests and forms and asks the ones in the session's tree. An ask that is listed and also arrives as an event is asked once.
Docs. The ACP page's "Child sessions" section is updated, and a new "Asks outside a prompt" section is added.
One event subscription per attached session now stays open between prompts. Before, it stayed open only while children ran.
How did you verify your code works?
bun test test/acp from packages/cli: 106 pass. New wire tests:
a continued child and the new grandchild it launches;
the session's own ask after the prompt;
pending asks at prompt start, with none from other trees;
pending asks after session/load.
The background-child test now also expects asks after the child ends. All six failed on the old code. Two fold tests cover an ask that is both listed and seen as an event, and asks from adopted descendants.
bun test test/acp/turn.test.ts test/acp/translate.test.ts --rerun-each 10: 560/560.
bun typecheck in packages/cli and bun run check at the root: pass. In services/www, bun typecheck, bun run check:generated and bun run build pass.
Live run.opencode acp ran from this branch, with its private server in simulation mode and a scripted model, driven by a real ACP client over stdio. Only model output was scripted.
Prompt 1: the root launches general "Level one", which launches "Level two", whose shell command asks for permission.
Prompt 2: the root continues "Level one" by session ID, and it launches "Level two again", which asks.
On this branch, both asks reached the client with depth 2, the second under the continued "Level one", and both prompts finished. On main, prompt 2 hung: the permission request from "Level two again" never reached the client, and the client gave up after 60 seconds.

Summary by CodeRabbit
New Features
Permission requests and questions from child sessions—including descendants and sessions continued from earlier prompts—continue reaching the client between prompts.
Pending requests are delivered when a session is loaded or resumed, before a new prompt, and after a prompt ends. Requests still displayed when a prompt starts are canceled and reissued.
Requests are linked to a tool call only when that tool call was previously sent to the client.
Documentation
Updated ACP guidance on child-session notifications, request routing, and pending requests.

📝 Walkthrough
ACP follows attached sessions and descendants between prompts. It delivers pending permission requests and forms, including after session load or resume. Tool-call references are included only when the client has received the tool call.

Request delivery remains active for longer and includes previously undiscovered descendants. The reviewed paths retain session-tree checks, explicit approval and cancellation safeguards. Remaining uncertainty concerns request identity and lifecycle edge cases rather than a demonstrated authorization bypass.
Retained concerns
No architecture-level concerns identified.
Security review details
Security Blast Radius
observed — The delivery scope expands temporally and includes validated descendants from earlier prompts. Approval consequences can exceed the individual child: existing always-allow behavior saves rules at project scope and reevaluates matching pending requests. This PR exposes additional asks to that existing approval mechanism rather than introducing new grant semantics.
Trust Boundaries and Controls
observed — Permission replies carry the original request session, and the server verifies that the request belongs to that session. Settlement events are produced from stored request ownership. Generated ask IDs include random bytes, and duplicate active permission IDs are rejected.
Resilience and Maintainability Implications
observed — Follower interruption leaves the server ask pending instead of submitting a fallback decision, allowing the next prompt to replay it. Explicit prompt cancellation instead races through reject/cancel outcomes. Outbound ACP requests receive cancellation signals.
Hardening Proposals
proposed — Apply session-tree membership checks consistently before recording settlements, and document or enforce ask-ID nonreuse for the subscription lifetime. This would reduce dependence on stream-wide identity assumptions; the reviewed evidence does not establish an authorization bypass.

Preserve source tool association when recovering permissions
packages/cli/src/acp/permission.ts:52
Permissions recovered after session/load have no tracked input.tool, even when history replay sent their source tool call. This therefore uses the permission ID and presents the permission as a different tool call. Fall back to request.source.id before the permission ID so recovered permissions retain their source association.

Keep background following active after submission failures
packages/cli/src/acp/turn.ts:500
run() has already stopped the previous follower. If submit() fails, this branch closes the replacement subscription without restarting following, even though the session remains attached and its descendants can still run. Subsequent permissions and questions then have no listener until another prompt or load. Preserve background following after submission failures; distinguish these from failures of the event subscription itself, which may need reopening. Add a regression with a failed submission followed by a background ask.

Cancellation and concurrent reconciliation can leave pending asks unseen or display settled questions that block later asks.

Cancelled initial scan loses pending ask handoff
packages/cli/src/acp/turn.ts:113
If a prompt is cancelled while connected() or the initial askPending() waits for I/O, stopFollowing() has already cancelled any open between-prompts dialog. Handoff preserves placed: true, so the replacement follower skips reconciliation. An ask created before this subscription then remains pending on the server without a client dialog. Initialize placed to false; only a completed scan should mark it placed. Add a regression that cancels a prompt during its initial scan with an existing pending ask.
Rescan race can display an already-settled question
packages/cli/src/acp/turn.ts:382
uninterruptible does not serialize a background rescan with advance(). After pending() records forms A and B, interpreting unsupported form A can await cancelUnshown()'s HTTP request. If B settles during that wait, AskSettled finds no deferred because B has not been queued yet. This loop then queues B anyway, showing an already-settled question that can block subsequent asks. The settlement history only prevents settlements before the fold, not this gap after it. Register settlement handles atomically with the state change and defer network cancellation, or serialize reconciliation and settlement handling. Add a regression that settles B while A's cancellation is delayed.
The Nix package's bundled opencode moves from 87e9437 to 005672f, adding ACP following of nested subsessions (developing-today/opencode#58), nested asks at the root in the OpenCode app (#60) and in the TUI with whole-subtree discovery (#64), plus the smaller merges listed in the PR.

What does this PR do?
opencode acpruns OpenCode behind an ACP client such as Zed. The client sees one session, and OpenCode routes that session's subsessions and their asks (permission requests and questions) to it. Until now ACP followed only subsessions created during the current prompt, and only until they finished. With nested and continued subagents, that drops asks, and the asking session waits forever:subagenttool withsessionID) emits nosession.created, so ACP never saw it or anything it launched.
