Summary
This patch stabilizes the Claude and Codex refresh paths under large local histories. Codex refresh triggers now share one Extension Host lifecycle with a strongest-pending follow-up, index teardown fences queued work until the replacement provider is ready, and project/status aggregation remains bounded.
It also makes incremental Claude content analysis preserve the established full-scan attribution and ordering semantics while keeping unchanged and ordinary append refreshes bounded. Live Webview patches retain safe UI state and local scroll positions, Claude calendar ranges roll over without rereading JSONL bodies, watcher recovery remains capped, and advice preview/consent state stays host-authoritative.
Fixes startup for installations with a legacy workspace-scoped advice API key or unavailable SecretStorage. The usage status bar, commands, and dashboard continue to activate; AI advice remains unconfigured until the key is manually migrated. The legacy plaintext key is left untouched and is never copied into a global secret slot.
