Which open-gpu-kernel-modules version are you running? Be as specific as possible: SHA is best when built from specific commit.

Which OS are you running this on? (output of lsb_release -d)

Which kernel are you running? (output of uname -a, say if you built it yourself).

Please confirm you are running a stable release kernel (e.g. not a -rc). We do not accept bug reports for unreleased kernels.

Please show what command(s) you ran that led to the issue.
