Yea that font size option is a dumb thing, and does not take the control dimensions into account. I would love to remove that option, as larger fonts (plus fixed control dimensions) are already supported through high DPI settings. That would be the recommended way anyway. Did you try a high DPI setting in Windows already?

Excuse my ignorance, how do I "Did you try a high DPI setting in Windows already?". This is something I can set in HeidiSQL or is it Windows settings?

Windows 11 has a DPI based scaling option in Settings > Screen: https://winbuzzer.com/wp-content/uploads/2023/10/00.2-Windows-11-Settings-System-Display-Scale-150.jpg

HeidiSQL supports that DPI scaling. Only do not combine it with the above mentioned font size, reset the font drop-down to "default" first.

A, scaling, yes. First, I reported versions as 12.21.0.7344 Windows 10. Second, even on Win 11 it defeats the purpose, I don't want to scale my whole OS for one app. As a developer, I need Full HD for example to be Full HD and report 1920x1080 as default resolution.

No worries, other apps have the same problem. But as I get older and my eyes get weaker, I really miss settings to separate font sizes of UI and content.

That's what DPI scaling does. Not all but most Windows apps nowadays support that, and the result is what I think you expect. However, that is the generally preferred method for making screens readable for older eyes. Here's a screenshot from the same dialog as you posted above, on Win11 at 150% DPI:
